The joint nobody trains

Five minutes a day for three days, with a sink and a rolled facecloth.

Women with hand osteoarthritis average under 60% of the grip strength of women without it. Nothing in an ordinary day loads the hand hard enough to hold that line, and no gym machine covers it either. So the hand is the joint most people let go quietly, and the one that takes the least effort to work.

The biggest test of hand exercise ran across 17 NHS clinics in the UK and enrolled 490 adults with painful, stiff rheumatoid hands. Half carried on with usual care. Half added a tailored stretching and strengthening program: six short sessions with a therapist, spread over 12 weeks, plus a daily set at home. At a year, hand function on a 100-point scale had risen 7.9 points in the exercise group against 3.6 points in usual care. Pain scores did not go up, and the trial recorded no adverse events.

For osteoarthritis hands the picture is smaller. A Cochrane review pooled seven trials and rated the evidence for short-term grip strength moderate, while the effect on pain was modest and rated low quality. Grip is the outcome most likely to move first. I would plan for that and treat any drop in ache as a bonus.

Nothing in that program needs equipment. Full-range finger movement, a squeeze against something soft, thumb-to-finger touches. Three days is enough to find out how your hands take it.

Put a clock on the stiffness first. Osteoarthritis stiffness normally loosens within 30 minutes of getting up. Stiffness that runs past an hour on most mornings, or knuckles that are puffy and warm on both hands at once, points at inflammatory arthritis instead. That is a doctor’s appointment before a grip program, because the medication side matters more than the exercise side.

 
✅ The 3-Day Plan

Baseline, right now. Make the tightest fist you can and see whether your fingertips reach your palm. Note which hand falls short. That is the number you are chasing.

Day 1. Hold both hands in warm water for three minutes. Then 10 slow fists per hand, closing all the way and opening to fully straight fingers. Then 10 tabletop bends: knuckles bent to a right angle, fingers held straight. Stop there.

Day 2. Repeat Day 1, then add the squeeze. Roll a facecloth into a sausage, grip it for three seconds, release, 10 times per hand, two rounds.

Day 3. Repeat Day 2, then touch your thumb to each fingertip and back, three rounds per hand. Finish by remaking that fist and checking the gap against your baseline.

More Hand Wins
Warm the hands, then move them. Cold fingers reach less range, so the set gets shorter without you noticing. Run the hot water while the kettle boils and do your three minutes there.
Fatten the handles you use most. A thin pen, a thin peeler and a thin toothbrush all force a harder squeeze for the same job. Slide a foam tube over the pen and buy the wide-handled peeler this weekend.
Break the long holds. Twenty minutes on a phone or a steering wheel keeps the fingers locked in one shape. Flatten your palm against your thigh for 10 seconds at every red light and every ad break.

“Won’t squeezing things every day wear my knuckles out faster?”

No trial has shown hand exercise speeding up joint damage, Ron. In the 490-person study the exercise group finished the year with pain no worse than the group that did nothing extra. Set your ceiling by the morning after: a knuckle that wakes up hot or swollen means the last session was too much, so halve the reps and build back from there. An ache in the forearm muscle is a different signal, and that one is fine.
