Stop letting your top knee drop in bed

One firm pillow between the knees takes seven hours of pressure off the outside of your hip.

Lie on your side with nothing between your knees and the top knee sinks toward the mattress. That angles the thigh inward and presses the gluteal tendons, which wrap the outside of the hip, against the bony point underneath them. Hold that angle for a whole night and the tendons stay squeezed the whole time. That compression is linked to gluteal tendinopathy, the most common cause of outer-hip pain after 50.

In a US study of adults aged 50 to 79, about 1 in 4 women and 1 in 12 men had pain over that bony point. The best-tested fix is the LEAP trial, published in the BMJ in 2018. It split people with this hip pain into three groups: advice on avoiding compressive positions plus home exercise, a cortisone shot, or wait and see.

Pillow between the knees, tonight. Use one firm pillow, folded in half if it is thin, so your top knee sits level with your top hip instead of below it. Knees and ankles both rest on it. Sleep with the sore hip on top, or on your back. Lying directly on the sore side presses the tendons from underneath.

● Stop hanging on one hip. Standing with your weight shoved into one side pushes that hip outward and loads the same tendons. In the next queue you stand in, keep your feet hip-width and weight on both.
● Stop sinking into a low sofa. A seat below knee height makes you lever yourself up off the hip. Add a firm cushion today so your hips sit level with or above your knees.
● Stop the cross-behind stretch. Crossing one leg behind the other and leaning sideways presses the outer hip hard. Replace it with 10 glute bridges: lie on your back, knees bent, lift your hips and hold 5 seconds each.

Drop any stretch that pulls the leg across your body, Linda. That position presses the outer-hip tendons, and the tight feeling there is often irritated tendon, not short muscle. The LEAP program steered people away from those stretches and gave them strengthening instead. Start with the glute bridges above, 10 a day, and keep the movement pain-free.
