Let your good arm do half the workFour seconds down, fifteen times, with a water bottle you already own. Pain on the outside of the elbow when you grip a screwdriver or a shopping bag is a tendon problem, not a joint problem. It peaks between 45 and 54, and only about one person in ten who gets it plays tennis.
The tendon involved is the extensor carpi radialis brevis, which starts on the outside of the elbow and runs down to the wrist. Every time you close your hand hard, that muscle fires to hold the wrist steady. That is why gripping is what hurts, and why weeks of rest change little. Rest lowers the load without changing what the tendon can take. One home exercise has a randomised trial behind it. Tyler and colleagues (2010) took 21 people with stubborn outer elbow pain, gave every one of them standard physiotherapy, and added a slow resisted lowering of the wrist for 11 of them. At about seven weeks the added-exercise group reported an 81% drop in pain against 22% for standard care alone, and their function scores separated the same way. The lowering is the part that counts. Your good hand does the lifting, the sore side does the slow return. That loads the tendon while it lengthens, which is the position it is weakest in, without asking the painful arm to produce the force twice.
Twenty-one people is a small trial, and it was stopped early because the gap between the groups was so wide. Treat the size as the limit on the claim, not on the exercise, which costs you about five minutes a day.

Turn the exercise over: forearm on the table palm up, lift the hand with your good side, lower it over four seconds, three sets of fifteen. A layer of foam or grip tape on the screwdriver, the mop and the frying pan means the hand closes less far to hold them, and a looser hand puts less pull through the outer elbow. Pipe insulation from the hardware shop costs almost nothing and cuts to length with scissors. |
