The exercise that emptied a surgery waitlist

Ninety-seven people were booked for a shoulder operation. Most who tried three specific moves first cancelled it.

Subacromial impingement is the pinch you feel reaching for a top shelf or twisting to check your blind spot. Doctors usually point at the joint. The three-day plan below points at the shoulder blade instead, the piece that is supposed to rotate up and out of the way and often does not.

In a Swedish trial, 97 people on the waiting list for arthroscopic shoulder surgery split into two groups. One group learned three specific moves: rotator cuff strengthening, scapular retraction, and a controlled overhead reach. The other group did generic range-of-motion work. Both groups kept their spot on the list and could take the surgery whenever they wanted, no waiting required.

A year in, 63% of the generic-exercise group had gone ahead with the operation. In the specific-exercise group, only 24% had. Same shoulder, same waiting list, a different set of moves.

The trial ran the exercises for twelve weeks. You do not need twelve weeks to feel the shift. Three days of the same movement pattern changes how your shoulder blade moves the next time you reach overhead.

Know the difference before you start. A dull ache while the muscle works is normal. A sharp catch under the point of the shoulder is not. If you feel the second one, drop the range and keep the reps small until it clears.

Day 1, right now: stand tall, pull both shoulder blades down and back like tucking them into your back pockets. Hold 5 seconds, release. Ten of those. Day 2: add a resistance band pulled out to the side at waist height, 15 reps each arm. Day 3: add a slow overhead reach with the band, pausing right at the spot where the pinch used to start.

More Shoulder Wins
Drop the overhead press for now. Rows and chest presses are fine. Anything that forces the joint into that pinched position at the top of a rep keeps stirring it up.
Sleep on your back or the good side. Lying on the sore shoulder compresses the same space the exercises are trying to open.
Warm up the band before you load it. Ten slow reps with light resistance before the working set gets the scapula muscles firing before they take heavier load.

“I’ve been on the surgery list for four months. Is it too late to try exercise first?”

No, Denise. In the trial, everyone stayed on the waiting list the whole time they exercised, so trying the moves costs you nothing but the weeks you are already waiting through. If your baseline pain is severe or you have a full-thickness rotator cuff tear on imaging, the odds tilt more toward surgery helping. Short of that, three months of the right moves is worth doing before the operation, not instead of your spot on the list.
