Does It Work?

The foam neck stretcher is not cervical traction

It has over 93,000 Amazon reviews, costs $16, and a bath towel does the same thing for free.

Foam neck stretchers have generated over 1.4 million posts on TikTok. The category leader, a curved pillow you lie on for 10 minutes a day, sells an estimated 7,000 units a month on Amazon. The listing calls it cervical traction.

How do your knees feel after a long stretch of sitting?

🟢  Fine 🟡  Stiff when I stand up 🔴  They ache the whole time

Reilly and Martens measured the force between the kneecap and the thighbone during everyday activities in 1972: 0.5 times body weight on flat ground, 3.3 times on stairs, 7.6 times in a deep squat. A desk chair holds the knee at roughly 90 degrees for hours at a time. Stairs hit harder per step, but each step lasts about a second. Sitting holds a smaller force steady for the entire stretch.

Patellofemoral pain affects about 23% of adults annually, according to a 23-study review (Smith et al., PLoS ONE, 2018). In UK general practice data from 2006 (57,555 registered adults), one in six knee consultations was coded as a patellofemoral disorder. The clinical trigger is not a sudden injury but hours of flexion without a position change.

In one small trial (30 patients with knee OA, aged 45 to 70, Journal of Clinical Medicine, 2022), isometric quad work reduced stiffness over four weeks. The extension position is the swap.

Stop if it pinches. A mild stretch at the top of the extension is normal. Sharp pain behind the kneecap during the move means stop and try a shorter range. Swelling with warmth and no clear injury needs urgent referral (AAFP, 2018).

✅ The 10-Second Swap

Do one right now. Sit with your feet flat on the floor. Straighten one leg until the knee is fully extended. Tighten the muscle on the front of the thigh (the quadriceps) and hold for ten seconds. Lower the foot. Switch legs and repeat. Set a phone timer for 20 minutes. When it goes off, do another round. No equipment, no standing up, no noise.

More Knee Wins
● Check your chair height. If your knees sit higher than your hips, the flexion angle passes 90° and kneecap load climbs. Raise the seat or add a firm cushion until your thighs slope slightly downward.
● Slide your car seat back. One notch back drops the knee angle and reduces kneecap compression during your commute. Adjust the position before you start the engine today.
● Prop your feet up tonight. Rest your feet on a low ottoman or stool while watching TV so the knees stay closer to straight, reducing sustained kneecap load.

Where do your knees spend the most time bent?

🪑 At a desk   🚗 In the car

From the Mailbag

“I get stiff knees after sitting but no pain when I walk. Is that still a problem?”

Walking loads the kneecap briefly with each step, then releases it. Sitting holds the load in one position for hours, which is why the stiffness shows up in the chair and not on the sidewalk. Start with the periodic extension breaks in the Fix above. If stiffness persists past a few weeks or gets worse, an orthopedic or sports-medicine clinician can evaluate the cause.

P.S. If you work at a desk, raise your monitor until the top of the screen sits at eye level. A ream of paper or a book under the base works. This keeps the head near neutral instead of angled forward all day.
