Stop Doing This, Try This

The pillow mistake that stiffens your knee by morning

Six hours of flexion each night shortens the tissue your knee needs to fully straighten.

Flexion contracture is the clinical term for a knee that no longer fully straightens. Over a third of people with knee osteoarthritis have it to some degree, and sleeping with a pillow under the knee is one of the habits that makes it worse over time.

How do your knees feel when you first stand up in the morning?

🙂  Fine 😕  A bit stiff 😢  Locked up

The joint capsule (the tissue sleeve around the knee) is at its slackest around 25 degrees of flexion. A pillow props the knee right there, and the ache drops fast. The problem is duration. Six to eight hours of sustained flexion gives the posterior capsule and hamstrings enough time to begin shortening. Over weeks, the knee gradually loses degrees of extension it needs for a normal stride.

In a 2025 multi-center study of 853 knee OA patients, 36% had some degree of flexion contracture. An analysis of the Osteoarthritis Initiative cohort (4,796 participants) found that even mild contracture, 1 to 5 degrees of lost extension, was linked to worse pain, worse stiffness scores, and slower 400-meter walk times. The association was severity-dependent: more lost extension, more symptoms (Campbell et al., 2018).

Bladder Leaks? MD Reveals Her #1 At-Home Fix

This MD’s 7-Second Harvard-Backed “Bladder Restoration” Trick works better than Kegels, vaginal cones, and bladder training.

Now I’m no longer afraid to wear light pants, dance, or be intimate with my husband out of fear of a ‘little accident’.

Life has never (ever!) been so good. Better yet, the doctor who uncovered this secret also posted a video, that’s free for the next few days, where she shows you what to do.

Don’t wait:

A knee that cannot fully extend at heel strike forces the quadriceps to work harder with every step. A few degrees sound small, but over thousands of daily steps the extra demand adds up.

Stop: Propping a thick pillow under your bent knee every night.

Try: A rolled bath towel, about 4 inches in diameter, under both ankles. The knee floats into a near-straight position. Gravity eases it toward extension without forcing it against bone. If full extension hurts, move the roll under the knee so it keeps a slight bend (10 to 15 degrees). Physical therapists use this swap after knee replacements to prevent contracture. The principle is the same for an arthritic knee: the capsule shortens in response to sustained position, implant or not.

Ease into it. If your knee hasn’t been straight at night in months, the first few nights with the towel may feel unfamiliar. Start with a thicker roll and thin it down every few days. A low-grade stretch behind the knee is fine. Sharp pain means back off, reduce the padding, and try again.

✅ Tonight’s Fix

Swap the pillow now. Roll a bath towel to about 4 inches thick and set it at the foot of your bed. Before you turn out the light, slide it under your ankles and let your knees settle. Rate your stiffness getting out of bed the next morning, 1 to 10. Do this three nights in a row and compare.

On the Wire
● Morning heel slides. Before you swing your legs out of bed, slowly bend and straighten each knee 10 times flat on the mattress. This pumps synovial fluid across the joint surfaces and may ease first-step stiffness. Try 10 per leg in the morning.
● Ice after activity, not before. A cold pack for 15 minutes after a long walk or garden session may help control the swelling that drives your knee toward flexion at night. Keep a gel pack in the freezer for tonight.
● Prone knee hang. Lie face-down with your lower legs off the edge of the bed, knees at the mattress line. Gravity gently pulls the shins toward straight. Hold 2 minutes. Physical therapists use this to recover the last few degrees of extension.

“My knee is too stiff to straighten all the way. Won’t forcing it flat make things worse?”

Ron, 66

Good instinct, Ron. The towel under the ankles doesn’t force anything. It lets gravity pull the knee gently toward whatever extension it can manage right now. Start with a thicker towel roll so there’s still a slight bend, and thin it down over a week or two as the tissue loosens. If the stiffness is getting worse month to month, or you feel a catching sensation inside the joint, see your doctor. Osteophytes or a loose body can physically block extension and need a different approach.

P.S. Heat a damp towel in the microwave for 1 to 2 minutes, test the warmth on your forearm, and drape it over a stiff knee for 15 to 20 minutes before bed. The brief warmth may help the surrounding tissue loosen enough for the joint to settle into a straighter resting position overnight.
